We have over 10 years experience in providing specialised Physiotherapy & Exercise Physiology to the aged care and disability population. We consider the uniqueness in each and everyone of those we help. We look at their circumstances and complexities to determine how we individualise our services to meet their needs.
We have and will continue to refine our services to not only provide individualised treatment but to ensure participants receive holistic & collaborative care. As we specialise in NDIS & Home Care therapeutic support, we understand the processes involved and how important it is to collaborate with the many providers involved. We work closely with support coordinators, case managers, plan managers, social workers and other allied health professionals.
The main goal of Colab Rehab to continually provide comprehensive & collaborative treatment across the health care system. It is one of the main reasons we provide mobile services, it helps us to bridge the gaps within the community and have our participants maximise their potential and capacity in life.
We have worked alongside HomeCaring for over 5 years and have built a coherent relationship with how we care for our participants. Over those years not only have we run workshops and engage in community events together, we have collaborated to provide our participants the most comprehensive care possible.
We continue to grow together and adapt to the ever-changing aged care & disability population. This involves multidisciplinary action between our participants and therapists, case mangers, support workers and other professionals involved. Together, we have developed means of communication and procedures to empower our participants to independency whilst improving their quality of life.
